1 of two ways... put a hook on the leader and snell a hook on the end along with a loop on the other end... you have a hook with the leader just thru the eye of the hook and one snells. the other is free to slide.. or you can go a little farther and snell the slide hook onto the leader.. either way would work i would believe..

Geeze Jerry, I used a smiley face...it was half a joke...but in truth he did hook more than 1/2 his fish under the chin...no way is this a slight to Stellie...you know me, you know who I think are good fisherman and who are hacks.

Are both hooks baited with seperate pieces, same piece or sometimes 1 way sometimes the other?

I use the Belmar rig too. Both the snafu and the slider can be used with it...loop to loop connection.
